East Sussex Dementia Services

Sussex Partnership provides a range of dementia services in East Sussex. The team that works with you, and the services you are offered, will depend on your level of need. All our services consist of multidisciplinary teams with staff who specialise in the treatment of dementia.
Whichever service you use, we will encourage you to remain as independent as possible and to keep up regular activities as well as accessing available services and support. We will also provide support to family members or carers who are looking after you as needed.
Memory Assessment Service
We provide memory assessment and diagnosis for people living in the High Weald, Lewes and Havens area, following GP referral. The team is based at Newhaven Rehabilitation Centre, and also has clinics at Grove House in Crowborough. (Other providers offer this service in other areas of East Sussex.)
Memory Support Service
Sussex Partnership and the Alzheimer’s Society are working in partnership to pilot the delivery of this service. It offers a range of interventions, such as memory management and cognitive stimulation courses, to help people feel confident about their ability to live well with dementia following a formal diagnosis. The service is not based in any one location but provides services in a range of community settings. Find out more here.
Living well with Dementia Teams (formerly Community Mental Health Teams)
This service is for people with dementia whose needs have become complex and challenging and referral is via your GP. We will assess your needs, and provide advice and treatment to help you manage your condition, including individual and group work, and psychological interventions. There are three teams:
- Hastings & Rother team, based at St Anne’s Centre, Conquest Hospital, Hastings
- Eastbourne, Hailsham & Seaford Team based at St Marys House, Eastbourne
- High Weald, Lewes and Havens team based at Newhaven Rehabilitation Centre (with a satellite hub at Highwood, Uckfield)
Extended Hours Team
This service is linked to the Living Well with Dementia teams and provides out of hours support at times of crisis.
Dementia Inpatient Wards
When people with dementia experience behavioural and psychological symptoms which cannot be managed in the community, there are two in-patient units which provide intensive assessment and treatment
- St Gabriel Ward at St Anne’s Centre, Conquest Hospital, Hastings
- Beechwood Unit at Uckfield Hospital, Uckfield
Care Home In-reach
This service goes into care homes and nursing homes with the aim of improving the quality of care in the home for people with dementia. They do this through providing training and coaching for the staff on person-centred care and by reviewing the treatment and medication of individual patients.
The Mental Health Liaison Service
This service provides support and advice to general hospital staff at Eastbourne District General Hospital and the Conquest Hospital in Hastings on how to help people with dementia and mental health conditions on the wards.
