Censeo patient questionnaire

Censeo is an online questionnaire for patients which allows you to share, in your own words, why you are seeking help.

Censeo won't replace our routine processes, it offers a digital healthcare tool to provide information to support services with triage and assessment, and provides you with increased accessibility and choice regarding how you provide information.

We are currently piloting Censeo in the Horsham Assessment and Treatment Service (ATS) for routine referrals. Completing the questionnaire is optional but is a helpful tool for tailoring your care. If you choose not to complete it, you will still receive the support you need. 

After the pilot, we plan to make Censeo available across all ATS services at Sussex Partnership. Other NHS trusts have reported that Censeo has helped reduce the need for service users to repeat their story to clinicians' multiple times. 

How Censeo works

Once we have received a referral from your GP, our team will assess whether you meet the criteria for our service and Censeo pilot. If your referral is suitable, you will receive a questionnaire via a secure text message link. Not everyone will receive a questionnaire as it is dependent on your symptoms and the support you require.

You can complete the questionnaire on a mobile phone, or copy the link into a browser on a laptop, desktop, or tablet. You will be guided through questions which will help to build a detailed picture of your mental health symptoms and concerns.

Censeo analyses information related to mental health including:

  • Mood disorders
  • Anxiety disorders
  • Psychotic disorders
  • Obsessive compulsive disorder
  • Trauma
  • Emotional instability
  • Adjustment disorder
  • Substance & alcohol misuse
  • Disordered eating
     

Note: It does not assess neurodevelopmental conditions or mental health symptoms for individuals under 18 years old.

Completing the Questionnaire

Because of the unique way Censeo is designed, the questionnaire is tailored to the individual. One person will be asked a different set of questions to another, depending on their needs.
Please be open and honest with your answers to help us understand the challenges you face.

The questionnaire typically takes one hour to complete, but this can vary from twenty minutes to two hours. You can save your progress and return later if needed. Once you have completed the questionnaire, your answers will be sent to the Assessment and Treatment Service team to review.
