When you receive services from us we may collect information about you that helps us to provide your treatment and care. Under The General Data Protection Regulation and Data Protection Act 2018 you have a right to request access to your health records. This is called a Subject Access Request.
You do not have to give any reasons as to why you are requesting your personal information and there is no charge for this service.
We have one month to comply with a Subject Access Request. If we require further information or clarification from the requestor, this timeframe is put on hold and restarts as soon as clarification is received.
Your personal record holds important information about your health, including medical history, medications, current prescriptions, allergies, assessment results, care plans and more. It will may also include the required personal information we need for our records, including name, date of birth, address, contact phone number and next of kin contact details.
