Learning from Deaths Policy
The purpose of this policy is to ensure all unexpected physical health related deaths are reported and investigated appropriately and proportionately to their level of severity. The Trust will implement the requirements outlines in the Learning from Deaths (2017) framework as part of the Trusts existing procedures to learn and continually improve the quality of the care provided to all patients.
