beseen by Psyomics is a secure, online questionnaire offered to adults referred to Neighbourhood Mental Health Teams (NMHT). The beseen questionnaire allows you to share, in your own words, what you are currently experiencing and why you are seeking help.
beseen won't replace our routine processes; it offers you the opportunity to provide information to support our service staff with triage and assessment. It provides you with increased accessibility and choice regarding how you provide information and an opportunity to prepare for your assessment.
Completing the questionnaire is optional but can be a helpful tool in tailoring your care and helping you prepare. If you choose not to complete it, you will still receive the support you need.

Once we have received a referral from your GP, our team will assess whether you meet the criteria for our service and for beseen. To be eligible, you must be aged 18 or over and have a personal mobile phone to receive the secure link. Not everyone will receive a questionnaire as it is dependent on your symptoms and the support you require.
Currently beseen is only available in English and there is no requirement to complete it. If you require an interpreter for your assessment, this will be arranged for your face-to-face appointment.
If your referral is suitable, you will receive a text message with a secure link to the beseen questionnaire. After the initial text message, reminder messages are sent on day 2, 4 and 6, if you have partially completed or not started it yet. Once you submit the form, these reminders will stop.
You can complete the questionnaire on a mobile phone or copy the link into a browser on a laptop, desktop, or tablet. You will be guided through questions which will help to build a detailed picture of your mental health symptoms and concerns.
The questionnaire typically takes 40 to 60 minutes to complete, but this can vary. You can save your progress and return later if needed. Once you have completed the questionnaire, your answers will be sent to the Neighbourhood Mental Health Team (NMHT) to review prior to your booked assessment appointment.

The beseen questionnaire includes validated outcome measures. Outcome measures are a series of questions used to assess a person's current state, which if completed again at a later date can determine whether any change has occurred. A measure is validated when research provides evidence that its scores are accurate, consistent, sensitive to change, and interpretable for the specific population and purpose it's being used.
The questionnaire will ask you to complete validated outcome measures to provide an overview of your current state and the symptoms you are experiencing.
Patient Choice: You can identify which areas are relevant to you and complete only those questions, and also choose to skip certain sections of the questionnaire if you feel they are not relevant to you.

Some people may find some of the questions challenging, difficult or upsetting to answer. You can take breaks, and your answers will save. You may also want consider who you could seek support from if you find the questions difficult to answer.
There is also an Urgent Help button visible at the top of the questionnaire screen. Clicking this will provide you with immediate support options and crisis numbers.
