How to make a referral

To make a referral please contact your local team via phone or email. Telephone calls are preferred as this allows us to get a better understanding of the person and the difficulties they are experiencing before we contact them directly for more information and to progress the referral.

Our contact details and referral form can be found here

What to expect after a referral is made

Our aim is to see people who are referred to the service within two weeks, but we try to see them sooner if possible.

The assessment process lasts up to six weeks and in that time the individual may see a number of different people in the team to make sure that we are completing the assessment thoroughly. After the assessment is complete we will determine whether we are the right service for the individual and if we are not, we will ensure that they are signposted to the most relevant and appropriate service for them.

We are able to offer support for up to three years.
