Primary Care and Partnership Mental Health Practitioners (PCP)

The Primary Care and Partnership Mental Health Practitioner team (PCP) is part of a wider team that includes:

  • Emotional Wellbeing Service
  • Access Facilitator Service
  • Screen and Intervene
  • Community Connectors (Social Prescribers)   

The Primary Care and Partnership Mental Health Practitioner team had been working within NHS Talking Therapies teams until April 2026. The team consists of registered professionals such as Mental Health Nurses, Social Workers and Occupational Therapists. 

This team will support individuals by offering mental health assessments, including risk assessments, brief interventions, plus signposting or onward referrals to other services that best suit the needs of the patient.  

They will work with other Sussex Partnership Foundation Trust services, to include the core Neighbourhood Mental Health Teams (NMHT), and aligned services such as NHS Talking Therapies.
