Discharge appointment
What to expect at your discharge appointment with Sussex Gender Service.
The purpose of this appointment is complete your care plan with Sussex Gender Service and transfer your onward care to your GP. The aim of this appointment is to ensure your gender affirming care continues safely and you know where to get support in the future.
During the appointment, you and your Named Clinician will review your care plan with the service, ensure that each of your gender affirming care pathways have been completed and discuss considerations for your lifelong care. If you have not yet accessed a pathway and now wish to, we can discuss this with you at your appointment and update your care plan instead of discharging you.
You are welcome to have your appointment online via Attend Anywhere or face to face in St Peter's Brighton. The appointment will take approximately 30-60 minutes and will be with your Named Clinician.
Once we have agreed your discharge, we will write to you and your GP with a summary of your care plan with Sussex Gender Service and recommendations for your ongoing care, including what to do if you change GP in future. There will also be details of how to access specialist support for gender affirming care in the future should this be needed.
If you do not attend this appointment or decide you don't want this type of appointment, your Named Clinician will write a discharge letter, which we will send to you and your GP.
