Forget Me Not Unit

Forget Me Not is a specialist dementia intensive care unit designed specifically to care for people with dementia to meet their clinical needs.

The Forget Me Not Unit is for people with dementia who are expressing their distress in a way that makes it difficult to support them in their usual environment. The Forget Me Not Unit will provide them with immediate and expert assessment, treatment, and support in a safe environment.

Referrals for admission to our wards are made via clinicians in our specialist community or hospital liaison services.

We are committed to getting to know you and your loved ones so that we can help you feel comfortable at your time of need. If we understand who and what is important to you, your background, likes and dislikes, the help that you need to stay safe and well and how you express such things as happiness, sadness, pain, and discomfort, then we can make sure the care we provide is right for you.

The first few days and weeks of your stay will involve a number of different assessments and discussions with doctors, nurses, occupational therapists, and psychologists. We will use all of this information to work with you and/or your loved ones to create your personal support plan, which you will have a copy of. We aim to work closely with you, and your loved ones from day one and will ensure that we keep your stay with us as short as possible.

The Forget Me Not Unit is open 24 hours a day, seven days a week.

If you wish to visit, please call the unit to arrange a date and time. We ask that visitors avoid visiting during main mealtimes, which are 12pm - 1.30pm, and 5pm - 6.30pm.

When visiting the unit, a surgical mask should be worn to comply with current Covid-19 guidelines.

Free onsite parking is available and a public bus stop is located at the entrance of the Swandean Hospital site.

We are a multi-disciplinary team which includes doctors, a matron, ward manager, nurses, psychological professionals, occupational therapists, activity workers, health care assistants, administrators, domestic staff and volunteers.
